What COB Means
COB stands for come on body. What it covers, why it is the most widely listed of the finish options, and the three things it does not tell you.
COB stands for come on body. It is one of the most searched abbreviations in this business and one of the most widely offered, which makes it slightly odd that almost nobody explains it properly.
The short answer
COB = come on body. Finishing on her body rather than inside her or in her mouth.
That is the whole of it. It is listed on profiles because it is a preference worth stating, not because it is unusual or difficult.
Why it is the most commonly listed of the finish options
Two reasons, and both are practical rather than mysterious.
It carries the least risk of the options, and it requires nothing from her that the rest of the booking has not already involved. Where CIM and CIF are decisions somebody makes individually, COB is closer to the default that everything else is measured against.

The finish options, side by side

Notice that these are four separate listings rather than a sliding scale. Somebody who offers COB has said nothing at all about the other three, and the assumption that one implies the next is the single most common misreading of a profile.
What it is not
It is not a comment on protection. COB describes where things finish, not whether a condom was used getting there. That is a different question and OWO is the listing that speaks to it.
It is not the same as CIF. Face is listed on its own for a reason. Plenty offer body and not face.
It does not cost extra. If it is on the profile it is included in the rate on the profile. A figure that moves at the door is a signal, and how to pay covers what to do about it.
The health part, briefly
Of the finish options this is the lowest risk, which is part of why it is the most offered. Lowest is not the same as none: skin contact still transmits some infections regardless of where anything finishes, and broken skin matters more than most people think.

Asking for it
Check the profile, and if it is listed there is nothing to arrange. Most people say nothing beforehand at all, which is fine.

She can still say no
A listing is what she is open to rather than a promise. She can decline anything on the night without explaining herself, at this agency and at every other one.
In practice it seldom arises, because what is written on a profile is what somebody is comfortable with. The point of the listing is that it works both ways.
